Our staff looks forward to meeting you and discussing your goals. We strive to keep our appointments on time as scheduled. Prior to your appointment, we encourage you to fill out the patient forms that are located on this website so you can accelerate your check-in process. Please download, print, and complete the forms and bring them with you to your appointment along with your insurance card and co-pay. If you are unable to download our patient forms, then it is recommended that you arrive at your appointment 15 – 30 minutes early so you can have ample time to fill out the appropriate forms.

So we may serve you better please arrive 15 minutes early to appointment with your most recent insurance card and prepared for Co-Pay fee, Thank You!

FINANCIAL INFORMATION

Fees and payments
We make every effort to keep the cost of your medical care as economical as possible. You can help by paying your co-pay at the beginning of each visit. Other arrangements can be made with our office manager depending upon your special circumstances. You will be charged an extra $10.00 if we bill you for your co-pay.

Insurance patients
With a proper referral or authorization, we will bill your insurance for you. Please remember you are responsible for any deductible, co-pay, co-insurance, or any other balance not covered by your insurance policy. The co-pay is due at the time of each visit. If you carry a balance greater than 90 days, those accounts are referred to a collection agency, so we can concentrate on providing your health care.

If you are insured by an HMO insurance…
Contact your primary care physician to obtain a referral or authorization if one is required. Our practice is considered to be a specialist practice and often requires prior authorization. If you do not have a referral or authorization, you may have to pay for your visit at the time services are rendered.

If you are insured by a PPO insurance…
Whom we participate with, we will be happy to bill your insurance company for you with PPO insurances in our network. Please remember you are responsible for any deductible, co-pay, co-insurance, or any other balance not covered by your insurance policy. The co-pay is due at the time of your visit.

Cash paying patients
You are responsible for charges incurred at the time of each visit. We do accept cash, VISA, or Master Card. No checks please.

Disability forms, family leave forms, physical examination forms
All forms that have to be filled out and signed by Dr. Delgado will have a charge of $10.00. Please leave those forms at the front desk with our front office receptionist with instructions and payment. We are happy to discuss any questions or concerns you have regarding insurance matters and these documents. Please feel free to talk with our receptionist or our billing clerk.
