Frequently Asked Questions

Appointments

We see patients on an appointment basis. We request that you call in advance so that we can reserve a time for you. We know how valuable your time is, and we make every effort to see you promptly. When births or emergencies cause conflicts in scheduling, we notify you as quickly as possible. Please be sure we have your updated phone numbers. If you need to cancel an appointment, please call us 24 hours or more in advance so we may allow another patient to have that appointment time. Missed appointments or cancellations less than 24 hours in advance are subject to $25.00 fee. If the doctor is running late or is called to an emergency, you will be offered the choice of rescheduling or waiting. We will try to reschedule you at your convenience.   As a patient of Rosemary Delgado M.D.Inc you will be asked to see both our Physician and our Nurse Practitioner so that they both have the opportunity to meet with you.

Telephone Calls

We encourage you to call our office with any questions you have regarding your care during normal business hours Monday – Friday 9am- 5pm. Our knowledgeable office staff will be able to answer many of you questions directly or may relay your questions to Dr. Delgado or one of our Women’s Nurse Practitioners. They will return your call at their earliest opportunity. Most calls are returned at lunch or after office hours.

After Hours Telephone Calls

If you find it necessary to contact Dr. Delgado after regular office hours, the answering service will page the doctor or the physician on call. After Hours calls are reserved for Urgent Matters . Please let the answering service know the nature of your call, your call will be returned at the earliest opportunity. If it is a true emergency or you are patient in labor and can not wait for a call back, please go to the nearest emergency room, or if pregnant, go to John Muir Labor and Delivery.

Prescriptions and Renewals

Most prescriptions are written with automatic refills for one year or until you next annual examination is due. If you should need an extension on a prescription written by us, please call during regular office hours to request an extra refill until your next visit. Have the telephone number of the pharmacy you wish to use available when you call. Please do not call after hours or weekends for prescription refills, pain medication or antibiotics. Your medical chart is not available for review. If you need to be seen, please use the urgent care clinic at night or on weekends. If you are taking birth control medications or hormone replacement, please plan ahead to allow 2-3 days for prescription refills.

No Shows / Cancelled Appointments

If you cannot keep your scheduled appointment, please call our office at least 24 hours in advanced to reschedule.  This will allow us to offer that time to another patient.  Failure to give 24 hours cancellation notice or failure to keep your scheduled appointment is a no-show and may result in a charge of $25.00

Insurance Patients

Please bring your insurance card with you to your appointment.  If your insurance plan required an office visit co-pay, this will be collected at the time of service.  The co-pay cannot be waived by our office; it is a requirement placed on us by your insurance carrier.  You are financially responsible for any co-insurance, deductibles, out-of-pocket expenses, and non-covered services.  You will be charged an extra $10.00 if we bill you for your co-pay.  If you are a  member of a health plan that Rosemary Delgado, M.D participates with, we will submit a claim to your insurance company on your behalf.

If you are insured by an HMO insurance…

Contact your Primary Care Physician to obtain a referral or authorization if one is required. Our practice is considered to be a Specialist practice and often requires prior authorization. If you do not have a referral or authorization, you may have to pay for your visit at the time services are rendered.  Rosemary Delgado M.D. Inc is contracted with the following HMO products

  • John Muir Physician Network
  • Hill Physician Medical Group

If you are insured by a PPO insurance…

Whom we participate with, we will be happy to bill your insurance company for you. Please remember you are responsible for any deductibles, co-pays, co-insurance or any other balance not covered by your insurance policy. Co-pays are due at the time of your visit.  Please contact your insurance to verify that Rosemary Delgado, M.D. Inc is apart of your network.   We are not contracted with Blue Cross Select plans

Cash paying patients

Please contact our Office Manager, Nicole Philbrick at (925)937-1915 for all self pay price quotes.  You are responsible for charges incurred at the time of each visit. We do accept cash, VISA , Master Charge & Discover. No checks please.

Disability forms, Family Leave forms, Physical Exams..

A $20 fee will be charged for processing of each disability / FMLA forms.  Please allow 5-7 business days for us to complete the forms.   If you have any questions regarding your Disability / FMLA forms please contact Jackie Guerrero at (925) 937-9345.

If you dropping off a paper form to be completed please make sure that the following items are completed on your form;

  • Name
  • Date of Birth
  • Last day worked
  • First day of your disability
  • Last 4 of your SS#

State Disability Claims completed online- Visit www.edd.ca.gov

  • Click on the  EDD online icon
  • Click on For Claimants then SDI Online

Follow the on-screen instructions, at the end of completion you will be provided with a receipt # beginning with the letter “R” followed by numbers.  Please contact our office with the following information

  • Receipt # (EX: R1000000123456)
  • Name – how it was completed online
  • Last day worked
  • Last 4 of SS#

Medical Records Release

There is a $20 fee for a medical record request.  Payment for these records will be collected prior to records being released.  A complimentary copy of your records will be sent to the Physician of your choice.  Medical Records can take up to 7-10 to complete.
